Every day in Scotland, people leave hospital feeling scared and alone after a life-changing diagnosis. CHSS is on a mission to change that — and the Supported Self Management Training Facilitator is a vital part of that work.
This role sits within the charity’s Service Delivery team and plays a key part in supporting individuals to rebuild their confidence, independence and quality of life through the Supported Self Management programme. From helping someone return to everyday activities like walking to the shops after a stroke, to empowering others to reconnect with passions and hobbies, the Facilitator supports people to live life to the full.
About the Role
Based at home with regular travel throughout Edinburgh and the Lothians, the successful candidate will co-facilitate CHSS’s six-week Chronic Disease Self Management workshops, working alongside another trained facilitator. They will also line manage volunteers who assist in delivering these sessions.
The workshops are designed to help people with long-term chest, heart and stroke conditions — including those affected by Long Covid — to better manage their health. By helping participants develop practical tools, confidence and a peer support network, the programme enables them to take control of their wellbeing and daily life.

Who CHSS is Looking For
Applicants do not need to have a medical background or prior experience of delivering self management training — full training will be provided. What matters most is a strong belief in people’s ability to live well with long-term conditions, along with:

A positive attitude towards disability and long-term health

An understanding of the challenges faced by people with chest, heart and stroke conditions

Excellent interpersonal and communication skills

Confidence in delivering information and leading group discussions
Applicants must have access to a car and hold a current, valid driving licence. Travel expenses will be reimbursed at the appropriate mileage rate.
